A suggestion you neither accept nor decline is kept. Switch notes, close the app, come back tomorrow — it is still there, attached to the passage it belongs to.
This is deliberate. A rewrite you have not read yet is not a reason to be hurried, and an app that throws the suggestion away the moment you look at something else trains you to decide before you have thought.
The one thing that does clear an offer is editing the passage yourself. The offer describes words that no longer exist, and applying it on top of what you have just written would be the wrong change — ask again and it works from the new wording.
The rewrite lands in the Markdown file, and the write goes into history like any other. If you accept something and change your mind, the previous version is there.
